Question : Problem: Wiring pinout from smartjack to router

Telco did not extend the demarc.  I need to know the pinout to run a patch cable from the smart jack to either a block or to the router.  The telco left the drop and the 4 wire conversion, so I need to know what pins to connect to the NIU, and router
