Question : Problem: Please help me to correctly Identify a Legacy DEC 21140-AF based NIC

Greetings;

I have a 4-PORT NIC of unknown manufacture that I am trying to identify so that I can load the correct drivers for it and then properly configure it.  I have accumulated a lot of information about the card, but so far I have been unable to find out exactly which card this _really_ is, and generic Intel 21140-based drivers dont seem to be working _properly_ (the card sort of works with them but several things arent right).

Here is what I currently know about the card:
  -  By the dates on the chips, this card was probably made in 1997.
  -  32-bit PCI interface
  -  4 DEC 21140-AF chips
  -  1 DEC 21152-AB chip
  -  4 Davicom DM9101F chips
  -  Two of the four 21140 chips have a white jumper wire connecting them to the 21152.  The 21152 has an additional jumper wire going to a component on the motherboard.
  -  The MAC addresses assign this card to Asante, but the PCI ID identifies it as a DEC.  
  -  The backplane of the card is laid out with 8 LEDs and 4 ports as follows (top down):

If there is any additional information that I can try to get to help identify this old card, please let me know.  Once I know whether it is an Adaptec, DEC, Asante, or whatever and what model it is, I am hoping that I can find drivers and utilities for it and that I can figure out how to get all ports functioning as I expect.
As far as operating system goes, I am building a special testing machine that can be booted into nearly anything from NT351 to Server 2008.

DEVICE: 0009 => Fast Ethernet Controller

Not very helpful. However, based on the information you provided about the MAC, it is very likely an Asante NIC, as that one had both the DEC 21140-AF and Davicom DM9101F chipset. If it is an Asante, you should be able to find drivers here:

In the picture of the front of the NIC, it looks like it says PCBA and a string of numbers. However, I can't make it out in the picture. To the right of that it also appears to have a REV number. Can you provide those?
