Question : Problem: Modem name and Manufacturer to get drivers

Just recently formatted a friends computer and now am trying to get the modem to detect. All attempts at different com ports have been unsuccessful. No clue what the modem is except for these numbers on the modem itself.

My questions are:
Is there a program out there to help detect a modems name and manufacturer?
Is not being able to detect my modem also giving me the PCI communication device and pci multimedia audio device conflicts?
